Informed Consent Form

Please note that SFMA (both level 1 and 2) involve manual assessment and manual handling techniques. This includes hands-on assessment and body-to-body contact between participants in practical sessions and by instructors when performing demonstrations and assisting participants during practical sessions.

Your participation in practical sessions and demonstrations is entirely voluntary.

Techniques are explained and demonstrated prior to and during practical sessions. If you feel there is a risk of injury, please do not participate.

As part of the assessment process, instructors and other participants may ask about your medical history. Providing this information is entirely voluntary. However, if you do not provide this information, please refrain from participating in any demonstration or practical component that relates to these questions. This is for your safety and to ensure the instructors and other participants do not do further assessment that may cause you injury or distress.

We encourage you to withdraw at any time should you do not wish to participate in any part of the program for any reason at all. You are not required to provide a reason to the instructors or other participants.

I confirm that I have read and understood the Informed Consent information provided. I understand the nature of the manual assessment and physical contact involved, that any participation is voluntary, that I may decline or withdraw at any time, and that providing medical information is optional but may limit participation.
