What Changes in Weeks 7-8 After Achilles Surgery

Weeks 7–8 mark the transition into moderate loading.

The tendon is now entering early remodeling and can tolerate increased tensile stress — provided it is controlled. Walking typically remains in the boot, but strengthening work becomes more deliberate and measurable.

During this phase:

  • Double-leg heel raises progress in volume and control

  • Moderate external load may be introduced in seated positions

  • Lower-limb strength work advances

  • Proprioception and balance become more structured

This is where capacity begins to rebuild — without abandoning protection.

Building Moderate Concentric and Eccentric Strength

Calf strengthening becomes more progressive in this phase.

You will perform:

  • Controlled double-leg heel raises out of the boot

  • Gradual eccentric emphasis

  • Moderate, stable loading in seated positions

  • Strict limitation to plantar grade

The goal is not maximal effort. It is consistent, symmetrical, and repeatable load exposure that allows the tendon to adapt safely.

How Gait and Balance Continue to Progress

Walking remains protected in the boot unless surgeon clearance is given.

However, proprioception and weight-acceptance drills become more structured. You will begin:

  • Single-leg balance (booted)

  • Controlled weight shifts

  • Aquatic gait progression

  • Stability-focused lower-limb strength

The focus remains on controlled movement, not speed or propulsion.

What This Phase Includes

This 2-week block includes:

  • Structured strength sessions (3× per week)

  • Moderate-load double-leg calf raises

  • Split squat progression (isometric and short range)

  • Hydrotherapy integration (if available)

  • Proprioceptive training

  • Built-in progression checklists

It does not include:

  • Single-leg heel raises

  • Running or jogging

  • Plyometrics

  • Deep dorsiflexion stretching

Those belong in later phases.

When to Move to the Next Phase

You are ready to progress when:

  • Moderate calf loading is tolerated without next-day swelling

  • Double-leg heel raises are symmetrical and controlled

  • Full weight bearing in the boot is comfortable

  • Morning stiffness is stable or improving

The next phase introduces boot weaning and more advanced loading — but only once strength and tolerance are clearly established.
